What if taking insulin didn’t always mean reaching for a needle? A newer way of delivering mealtime insulin involves inhaling it through a specialised device instead of injecting it under the skin.
While the internet is calling it “insulin vape”, medically there’s an important difference: this is a medical inhaler that delivers insulin powder to the lungs, allowing it to enter the bloodstream quickly.
So, how does inhaled insulin work, how fast does it act, and can anyone with diabetes use it?

In a conversation with indianexress.com, Dr S Satish Kumar, HOD & Lead Consultant – Endocrinology, Diabetology & Bariatric Medicine, Aster RV Hospital, explains how it works, how quickly it acts and who should stay away from it.
How is inhaled insulin different from insulin injections?
“Inhaled insulin is a quick-acting insulin type which gets into the body through the lungs with the help of a specialised inhaler, rather than being injected into the fat under the skin like the traditional method,” Dr Kumar notes.
When inhaled, insulin reaches the bloodstream through the lungs, allowing it to act quickly on the rise in blood sugar that occurs after a meal.

“In contrast, insulin obtained through injections, which involve needles, is infused more slowly from the fatty tissue, while inhaled insulin works quickly and its effect is gone in no time,” the endocrinologist points out.
Inhaled insulin is primarily used before meals and does not replace long-acting or basal insulin, which many people with diabetes may still need in injectable form.
How quickly does inhaled insulin work?
The biggest advantage of inhaled insulin is its speed of absorption.
“Inhaled insulin is a fast insulin delivery method that gets administered right before meals, its purpose being to control blood sugar spikes that occur after eating,” Dr Kumar says.

The insulin powder is delivered into the lungs through a small, handheld inhaler and is absorbed into the blood rapidly. According to Dr Kumar, inhaled insulin usually works within 10–15 minutes, while injectable rapid-acting insulin takes about 30 minutes to attain its full effect.
The trade-off is that inhaled insulin has a shorter duration of action. Therefore, it cannot replace long-acting insulin injections that provide all-day control.

Who should avoid inhaled insulin?
Inhaled insulin may be an option for adults with type 1 or type 2 diabetes who require mealtime insulin. However, it is not suitable for everyone.
People with type 1 diabetes who use inhaled insulin still need long-acting injectable insulin. People with type 2 diabetes who require mealtime insulin may also be candidates, depending on their individual treatment needs.

“Smokers, those who have just quit, and patients with lung diseases like asthma, COPD, or chronic bronchitis should not inhale the insulin as it might aggravate their breathing problems,” Dr Kumar cautions.
The treatment is also not recommended during respiratory infections. Lung function tests are required before starting therapy and during the course of treatment.
Dr Kumar also advises against the use of inhaled insulin in children, pregnant women and people with unstable lung disease.
What are the side effects and limitations?
“The most common side effect of insulin inhalers is cough, throat irritation, or dry feeling in the chest which occurs mainly when the patient begins treatment,” Dr Kumar reveals.

Another limitation is dosing flexibility. Compared with injections, it may be harder to make very fine adjustments to the insulin dose.
“Inhaled insulin can only work for the short term; hence, they cannot take the place of injections of long-acting insulin,” he said.
There are also concerns around long-term lung safety, with Dr Kumar noting that there is still limited data on the subject. This is why lung function needs to be assessed before and during treatment.
Most importantly, an “insulin vape” should not be confused with a conventional vaping device. Inhaled insulin is delivered through a specialised medical inhaler containing insulin powder. It can offer some adults a needle-free option for mealtime insulin, but it is not a universal replacement for injectable insulin and is unsuitable for people with certain lung conditions or smoking histories.
